The difficulty in understanding the work of Tsitramon lies in the fact that for several decades of its existence in the domestic pharmaceutical market, it has undergone many changes. In the USSR, this drug was based on acetylsalicylic acid with phenacetin (0.24 and 0.18 g).

The latter substance is an analgesic with antipyretic properties, belonging to the category of anilines and capable of causing a weak euphoric state. The main disadvantage of phenacetin is its nephrotoxicity, especially if it is used in combination with other analgesic drugs. According to some studies, phenacetin is even the cause of the formation of tumors in the urinary system, so it was banned for use in medicine.

For this reason, the composition of the modern Citramon is significantly different from what it was in Soviet times. Now it only:

  • acetylsalicylic acid,
  • paracetamol,
  • caffeine.

Cocoa and citric acid, which were present in the tablets, were also excluded from the composition, and phenacetin was replaced by a safer, according to experts, paracetamol. It is related to phenacetin, since it is its metabolite, has the same analgesic and antipyretic qualities. It is noted that he has a weak anti-inflammatory properties, but the group does not belong to the group of NSAIDs for more than 17 years. The use of large doses of paracetamol can adversely affect the condition of the kidneys, liver and cause hemolysis.

Due to the fact that paracetamol is associated with phenacetin, it also has nephrotoxic and, in addition, a hepatotoxic effect, it can provoke the formation of methemoglobin, but to a lesser extent than phenacetin.
